  • One month's worth of pay stubs
  • SSI award letter
  • SSDI award letter
  • Veterans benefits letter
  • Latest tax forms (Form 1040, W-2, or 1099)
  • Statement of No Documented Income (if no income)

FOR THE PRIMARY APPLICANT

  • Government-issued photo ID

FOR ALL APPLICANTS

  • Recent Duke Energy (or other utility) bill showing 12 months of usage history (Look for "Your Energy Use History" on page 2)
